HIV Care Outreach Specialist - A full time qualified and committed professional for a part-time HIV Care Outreach Specialist. This position is responsible for conducting outreach and connecting HIV positive individuals to medical care. HIV Care Outreach Specialist primary responsibilities include ensuring patients remain in care; conducting follow-ups via phone calls, letters or visits for missed appointments, street outreach, and testing. The HIV Care Outreach Specialist must stay abreast with current trends and HIV-related research and services, especially in the health center’s service area.
